Existential-Humanistic Case Formulation

Louis Hoffman and Heatherlyn Cleare-Hoffman developed a new approach to case formulation in their book, Case Formulation in Existential-Humanistic Therapy (American Psychological Association, 2025). The template is available below. It is strongly recommended to only use this template in conjunction with the book.
